Step 1: Choose a Test Kit

We offer a range of test kits designed to help identify potential hormone and gut imbalances. These kits are simple to use and completed from the comfort of your home.

Step 2: Submit a Test Request Form

Once you’ve selected a test, click “Request This Test” to fill out a short, HIPAA-compliant form. This lets our team know you’re ready and gives us the details we need to follow up.

Step 3: We’ll Follow Up to Confirm Details

A member of our team will reach out to:

  • Answer your questions
  • Confirm your test selection
  • Collect payment
  • Arrange test kit pickup or shipping
  • Send a consent form confirming the cost, contents, and process

You’ll also receive a copy of the signed consent form for your records.

Step 4: Complete & Return Your Test

Follow the instructions included in your kit. Once completed, you can mail the test to the lab or drop it off at the pharmacy. We’ll notify you when results are received.

Step 5: Complete Your Health History Form

We’ll provide a link to a secure, detailed health history form. This helps our pharmacy team fully prepare for your upcoming consultation.

This form must be submitted at least two days before your scheduled consultation.

Step 6: Schedule Your In-Person Consultation

Once your test results and forms are complete, we’ll send you a link to schedule your visit.
